Community-based supports
This service is generally provided with a trained staff person assigned to work one on one with a person with disability.

The service can occur in any community setting including the person's natural home, group home, school, day placement (such as The Enrichment Center), etc.

This service is funded by Medicaid through the Community Alternatives Program (CAP) or Community Based Services (CBS). Community Bases Supports are “flexible” services that can be tailored to meet a variety of individual needs.
